Provides Primary Care Nurse Practitioner services in the office setting in a Patient Centered Medical Home model of care. Promotes patient wellness as well as a chronic disease management approach to care. Delivers targeted services to those with high risk disease states.
The Primary Care Nurse practitioner (NP), is responsible for providing clinical care within scope of practice for all patients of Scottsdale Healthcare in the primary care setting. Consistently demonstrates quality, service, professionalism, respect, compassion and effective communications with patients, families, clinic staff and management. The practice of the NP emphasizes health promotion, and maintenance, disease prevention and the diagnosis and management of acute and chronic diseases. The NP works as a team member in coordinating care of the patient population in the Medical Home model.
The position is associated with a significant level of autonomy; therefore, the individual must be highly motivated and able to work independently. As an advanced practice nurse (APN) must demonstrate leadership as a provider. The APN adheres to and functions within the scope of practice defined by the Arizona Board of Nursing, and Scottsdale Healthcare privileges.
The position requires sharing in call coverage for weeknights and covering call on weekends in rotation with other clinicians. The NP will be functioning with the appropriate support and supervision of the physician as dictated by state regulations policies of SHC.

Master's Degree (MS or MSN) from a National League of Nursing (NLN) approved institution. Arizona Nurse Practitioner license with prescriptive authority. DEA licensure. Board Certified.
